A copy of prior year’s Federal and State tax return, if any (if not prepared by us)
Federal or State correspondence received during the year, if any. List of any activities in other states
List of any corporate changes during the year. Details of any employee benefit plans including retirement plans, health insurance, etc.
Details of each business owner (Name, Social Security Number, Address,
Percentage of ownership/number of shares, date ownership acquired)
Additional owner details:
Date ownership given up, if any. Dates and amounts of distribution during the year
Details of any benefits received from the company (car, health insurance, etc.)
Tax Year's Financial Data:
QuickBooks data as a portable file for the current year
If QB data file cannot be transferred for any reason, please provide the following in MS Excel/PDF format:
Profit and Loss Statement Summary for the tax year (cash basis as well as accrual basis)
Profit and Loss Statement Details for the tax year (cash basis as well as accrual basis)
Balance Sheet for the tax year (cash basis as well as accrual basis)
If using an accounting system other than QB, please provide the following in MS Excel format:
Income & Expense Summary for the tax year; Income & Expense Details for the tax year; Balance Sheet for the tax year
If bank accounts are reconciled:
Bank reconciliation report for December of the tax year
Bank statements for each business bank account for January and December of the tax year
If bank accounts are not reconciled:
Bank statements for each month of the tax year

Fixed Asset Listing/Report
Details of fixed assets purchased during the year - Date of purchase, Description, Cost, Category (New/Used)

Details and Accumulated Depreciation Details
Details of any assets traded in or leased. Schedule of loans to/from owners including loan agreements
Details of any related party transactions. All loans/credit cards/lines of credit - document showing balance at year-end and interest for the tax year
Copy of W3 and all 1099s issued for the tax year, if any
Copy of 941s, 940 and state unemployment insurance reports, if any
Copy of Sales Tax Returns filed during the year, if any
Details of estimated tax payments - date and amount
Details of travel expenses claimed – per diem if any, lodging, airfare, etc.
